New Patients“ can now complete all of their New Patient Documentation from the comfort and privacy of your home prior to your visit by signing onto our Secure Patient Portal. Our staff will send you a USER ID and PASSWORD so that you may complete the patient assessment at your convenience.  When submitted this will enable our staff to review with you and improve the accuracy on your Medical Record which also  later can be reviewed by you on the Patient Portal whenever you want to review your progress. Please see the link to Patient Portal on our website and once you have been verified by our staff, your USER ID and PASSWORD will be sent. All at your convenience from the comfort of your home.

To Better Assist Our Establised Patients, we have established a Secure, HIPPA Compliant  Patient Portal to View & Update your personal information, address, phone number all demographics, emergency contacts, Refill Medications, Review Laboratory results and view Patient Education all from the secure, comfort of your home. Log onto the Patient Portal to receive your Secure and Private USER ID and PASSWORD. Easy and SECURE!!

Our Website and Patient Portal  are informational only and not intended to replace contacting our office for your medical needs. The Patient Portal is secure, safe & encrypted for information only to assist our patients with their healthcare needs and additonal Education. Never share your USER NAME OR PASSWORD WITH ANYONE to ensure your security.

Appointments:
We see all patients by appointment only. To make an appointment, please contact our office at (727) 466-6564 or you may log onto the Patient Portal and request a SECURE & SAFE Personal ID which will give you 24/7 access to your medical information, make appointments and other information from the comfort and privacy of your home!! 

Occasionally, emergencies arise and appointment times may be delayed due to another patients needs. We respect the importance of your time and our office will make every possible effort to care for you in a timely fashion.

If you cannot keep an appointment, please contact our office to notify us so that the time can be given to a needy patient. We request 24 hours notice for all cancellations.

Prescriptions:
All prescriptions will be filled and filed electronically to your pharmacy or whomever you direct our staff to contact during business hours only or you may update this information on your Patient Portal. Please notify our staff of any insurance changes which could effect your medications. Our office will not refill your medications after hours but you can update your Pharmacy Information and request refills on the Patient Portal from the comfort and privacy of your home.  We request that you advise our office of your medication needs at least 48 hours before your last dose to make sure your Insurance Carrier has enough time to process approval/authorization for your medication(s) needs.

EMERGENCY CARE:
In case of an emergency situation, please seek immediate medical treatment by calling 911 or going directly to the emergency room. Please ask the Emergency Room Physician to contact Dr. Shields or the Physician on call with your medical status. Our office is electronically equipped to receive this information. Please contact our office for follow-up medical care, after being released from a hospital admission. (normally within 7-10 days post release from hospital stay). The Patient Portal is informational only and NEVER to be used for emergencies. Emergencies should always contact 911 directly or go directly to the emergency room.

If the situation is not an emergency but needs medical attention, Please contact our office via our Patient Portal to request an appointment and our staff will contact you with confirmation within 48 hours or you may contact us directly at 727-466-6564 during  normal business hours. Our staff is trained and experienced to inform Dr. Shields of your needs and he will either return your call or instruct our office staff during normal business hours. After hours the answering service will contact Dr. Shields or the Physician on Call. Within 30 minutes, you should receive a call from Dr. Shields or the Physician on Call. If you do not hear within 30 minutes from the physician on call, please call the answering service again and ask to speak with the physician on call directly.
