Please contact our office within 2 or 3 days after you are discharged from the hospital. This “Transition of Care” appointment is very important for your continuity of treatment.
If your medical situation is not an emergency, please contact our office at (727)-466-6564 during normal business hours to seek treatment and our staff will gladly assist you. All billing calls should go to the billing office at (727) 729-4210 and ask for Cathy.
